Lessons from undocumented access: why ownership must be visible

Identity access governance visual representing ownership and reviews.
Undocumented access creates risk long before an audit or incident exposes it. Shared credentials, unexplained group membership, and accounts without owners make it difficult to know who can reach a service and why. A practical improvement is to assign an owner to every privileged group, service account, and application role. Record its purpose, approval process, review frequency, and recovery contact. Run access reviews on a schedule and remove access when the business need ends. Visibility is not bureaucracy. It gives support teams the information needed to resolve access issues safely, gives auditors a defensible trail, and gives the organization a way to reduce permissions without guessing.
